How it works
See your violations.Understand their impact.
Start with a free report. If you want to prepare a Request for Data Review, use the automated completeness check and review the fixed-template draft before filing it yourself.

Free lookup
Enter your USDOT number
Find your carrier’s inspection, violation and crash records in public FMCSA data. You do not need to upload documents to start.
We never ask for your FMCSA Portal, DataQs, or Login.gov credentials.

Free report
Read your free report
See the violation, its inspection date and out-of-service status, its weighted numerator contribution, the marginal change in the named BASIC measure, and how long the inspection counts. The lookup covers four BASICs; see the coverage details above. Where available, the report also shows your estimated position and its range.
The free report also has a crash section for the snapshot’s 24-month window and an MCS-150 line with the last recorded update and next scheduled deadline under 49 CFR 390.19T, including the conditional PRISM exemption. Free crash alerts and MCS-150 reminders are optional.
This is our estimate from public FMCSA data, not an official FMCSA percentile. Every report identifies its data snapshot.

Check your documents
For an inspection request, upload your inspection report and supporting documents. Review each extracted value alongside its source and confirm it.
Before payment, the software can verify three inspection bases: a documented paper difference in the report number or inspection date; a record-changing court outcome supported by an uploaded disposition; or a violation recorded in the federal record but not on your roadside inspection report, after you confirm the complete violation list.
For a CPDP crash request, choose the crash type, upload the police accident report (PAR) and supporting evidence, and confirm the required values. The eligibility and completeness check is free before payment. FMCSA alone decides preventability.
$79 USD before tax covers an automated completeness check and a fixed-template draft of your first Request for Data Review for one inspection and the selected violation in the case, or one selected federal crash record and crash type under CPDP. Paddle shows any applicable tax and your total before you pay. A different inspection, violation or crash is a separate purchase. Nothing is charged automatically.
The inspection review-stage calendar and inspection percentile comparison do not apply to a CPDP crash kit. We do not estimate a Crash Indicator percentile.
The automated category and completeness preview is free, before checkout. If no documented basis is verified, the answer is free and nothing is charged. Missing evidence and a document the software could not read are different results; each is explained separately.

You review and submit
Review and file your request
After payment, the software assembles your full draft from a fixed template and only the values you confirmed. Review the request, sign it and submit it through your own DataQs account.
You sign this request, not us. A false statement to a federal agency is a crime.
We do not submit the request on your behalf, provide legal advice or promise an agency outcome.
