Example
Weights depend on the inspection’s age at each monthly SMS snapshot. This example has a time weight of 3.
This inspection has 14 severity points before time weighting, below the 30-point cap within this BASIC. This violation contributes 12 of its 42 weighted numerator points; other violations contribute 30.
An automated completeness check and fixed-template draft of your first Request for Data Review for one inspection and the selected violation in the case, or one selected federal crash record and crash type under CPDP. A different inspection, violation or crash is a separate purchase. You file it in DataQs yourself. $79 USD before tax; Paddle shows any applicable tax and your total before you pay. Nothing is charged automatically.
Review each value alongside its source. Only confirmed values enter the draft.
See the software’s check of the selected Request for Data Review category and required evidence using available data and values you confirm.
See the dates that apply to your filing. The inspection review-stage calendar does not apply to CPDP.
The software assembles the draft from a fixed template and values you confirm. Review, sign and file it in DataQs yourself.
For an inspection request, upload the inspection report and supporting documents. For a CPDP crash request, upload the police accident report (PAR) and supporting evidence. Check each required value against its source and confirm it.
Nothing reaches your draft until you confirm it. A field you did not confirm cannot appear in the text — that is enforced in code, not by review.
For inspections, check the documented basis and required evidence. For a CPDP crash request, check eligibility and completeness for the selected crash type. These checks are free before payment.
Assembled automatically from a fixed template and only the fields you confirmed, carrying the signature warning and the 18 U.S.C. 1001 notice that the filing puts on you.
You sign this request, not us. A false statement to a federal agency is a crime.
We use public FMCSA SMS input files, published monthly. Every report names its data snapshot. If new data is unavailable or fails our checks, we show the previous snapshot with its date.
This is our estimate from public FMCSA data, not an official FMCSA percentile. Each available estimated position includes its range. A maximum error observed in a validation sample does not bound errors for every carrier or future snapshot. Matching a BASIC measure does not validate an estimated position, and passenger-carrier comparisons do not establish accuracy for property carriers. If the interval crosses the intervention threshold, the estimate cannot put the carrier firmly on one side of the line. SMS measures and estimated positions are not an official safety rating.
We omit positions that fail our checks and explain why. The free lookup covers Vehicle Maintenance, Hours-of-Service Compliance, Controlled Substances/Alcohol and Driver Fitness. Unsafe Driving, HM Compliance and Crash Indicator are not supported by this lookup; we do not promise their measure or estimated position. A reference code in the catalog does not mean a personal calculation is available.

The four supported BASICs require readable snapshot data and a computed measure to list violations or open a report. Estimated positions also require sufficient carrier data and passed publication checks; calibration failure can withhold a position or retain a dated previous estimate. Controlled Substances/Alcohol is subject to these checks too. Missing carrier data, no relevant inspections, no applicable violations and an unavailable calculation are different states. An unavailable result is not a zero or a claim that there are no violations. HM Compliance and Crash Indicator have no report calculation in this product.
Unsafe Driving’s SMS denominator is average power units × utilization factor, which depends on vehicle miles traveled (VMT) and carrier segment (Equation 3–1). Our product lacks the historical power-unit inputs needed to reconstruct and validate that full calculation. It calculates numerator points internally only; Unsafe Driving records, measure and estimated position are not offered in this lookup.
Severity weight is an input. Weighted numerator contribution accounts for time weighting and the inspection cap. The marginal change in a named BASIC measure also depends on its denominator. An estimated percentile compares carriers within a safety event group and must be read with its range. Do not add points across BASICs. SMS calculations are separate from an official safety rating. This is our estimate from public FMCSA data, not an official FMCSA percentile.
